I am an out of network provider for all insurance carriers (Premera, Regence, Aetna, Cigna, First Choice, Kaiser, United, etc). I do not take AppleCare or Molina. (My contract with Premera ended Jan 1, 2021 - if you see somewhere that I am in network with Premera, that is outdated information). 


I accept payment at each session, but am happy to submit courtesy claims for you. It is your responsibility to check your benefits to understand what your financial portion will be based on your plan and network. It is also your responsibility to determine if pre-authorization or a referral is required before we begin working together. 

The initial session is $250. Subsequent sessions are $225. I accept payment by cash, check, or credit card (Visa, MasterCard, Discover). I can also charge HSA/FSA cards. 

I require 48 hours notice for cancellations. I do charge my full session rate for appointments cancelled with less than 48 hours notice. I keep a credit card on file for this purpose.
